On-Line Failure Analysis Request Submission Form

The on-line failure analysis form should only be used after field application engineering has been contacted as an initial means of resolving the issue.

The on-line form should be filled out by Atmel direct sales personnel or sales representatives after all available resources have been exhausted (including the use of product support hotlines). If you are not a direct Atmel sales contact (with an @atmel.com address), it is a mandatory requirement that you carbon-copy an Atmel sales contact via the "other e-mail addresses that should receive notification" in Section 3 of the Failure Analysis form.

Please note that the on-line FA Form has been tailored to obtain information that is dependent on the failure mode.
Please attempt to provide as much information as possible, while, at the same time, not impeding the process of having the physical FA samples sent back to the appropriate Atmel locations as expeditiously as possible.

Once the form has been completed, you will receive an e-mail confirmation message with all of the information that was entered. Please print this message and include it with the FA samples that are sent back; this print-out will serve as the FA request form.
